چکیده

A 45-year-old man presented with an acute anterior wall myocardial infarction and underwent successful percutaneous transluminal angioplasty of the left anterior descending coronary artery. Twenty-four hours later, a wide QRS tachycardia at a rate of 108 beats/min that had a QRS duration of 150 msec was noted by telemetric monitoring. There were no symptoms associated with the tachycardia. A 12-lead ECG was recorded (Fig. I). The tachycardia terminated after infusion of 12 mg of adenosine (Fig. 2). What is the mechanism of this adenosineresponsive wide QRS complex tachycardia?

Wide QRS complex tachycardia in a patient with Wolff-Parkinson-White syndrome and cardiomyopathy: what is the mechanism?

A 32-year old woman with severe mitral regurgitation. congestive heart failure, and Wolff-ParkinsonWhite syndrome developed frequent, nonsustained episodes of a wide QRS complex tachycardia several days postpartum. Figure 1 shows an ECG with telemetry lead I and modified chest lead (MCL) recorded while the patient complained ot" palpitations. The same pattern of initiation and termination of th...

Wide QRS tachycardia. What is the mechanism?

A 70-year-old gentleman was referred for radiofrequency catheter ablation of wide QRS tachycardia that was not terminated with intravenous adenosine. Twelve-lead ECG was normal during sinus rhythm and transthoracic echocardiography demonstrated no evidence of structural heart disease. The electrophysiological study showed a normal AH interval of 104 m and HV interval of 45 m during sinus rhythm...

Clinical significance of wide QRS complexes at the termination of paroxysmal supraventricular tachycardias.

BACKGROUND A wide QRS complex is not a rare electrocardiographic phenomenon at the termination of paroxysmal supraventricular tachycardia (PSVT), but no plausible underlying mechanism has yet been proposed. The purpose of the present study was to elucidate the frequency and the underlying mechanism of the wide QRS complexes at the termination of PSVT. METHODS We retrospectively reviewed 305 e...

Widening of the QRS complex during atrial pacing: what is the mechanism?

An electrophysiologic procedure was performed in a 50-year-old man with a 20-year history of paroxysmal tachycardia. During sinus rhythm, the atrial-His (AH) interval was 70 msec, the His-ventricular (HV) interval was 45 msec, and the QRS width was 80 msec. During atrial pacing at a cycle length of 290 msec, there was widening of the QRS complex (Fig. 1). The transition from a narrow to wide QR...
